COMPLETING THE ESTA QUESTIONNAIRE
Citizens of countries participating in the Visa Waiver Program can apply for ESTA online. The questionnaire can be completed in just a few minutes, provided you have the required documents.
WHAT IS THE ESTA USA QUESTIONNAIRE?
The ESTA questionnaire is a form that must be completed in order to receive authorization to stay in the United States(ESTA Las Vegas, New York, Miami, Los Angeles…).
In order to complete it correctly, please make sure you have :
- your passport for the USA;
- payment card (processing fees can only be paid online).
The ESTA questionnaire has several sections. The first concerns the traveler’s personal information. First, you’ll be asked questions about your civil status: surname, first names, date of birth, nationality, passport number, date of issue and expiry, parents’ names, postal and e-mail addresses, telephone number… Then, if you’re employed, you’ll be asked to specify your employer’s company name, address and telephone number. You can also specify whether you use social networks (Facebook account, etc.). This question is optional, and you are under no obligation to answer it.
The second section concerns the trip itself, specifying the reasons for your trip, as well as the address to which you will be travelling.
The last questions concern your medical history (allergies, infectious diseases…), your criminal record (membership of dangerous groups, illegal residence on American soil, previous visa refusal…) and your past travels to Iraq, Iran, Syria, Sudan, Yemen, Somalia and Libya since March 1, 2011.
The questionnaire must be filled in carefully. Any answer that is incorrect or does not correspond to reality may result in a refusal by the American authorities. In this case, you’ll have to reapply after the refusal.

If you are a national of one of the countries covered by the Visa Waiver Program and are not also a citizen of Iraq, Iran, Syria or Sudan (dual nationality), you can apply for an ESTA online.
This takes you to the mandatory ESTA questionnaire, translated into French. Once you’ve filled in the form, check your answers one by one to make sure your questionnaire has been completed correctly and that nothing is missing. Your ESTA application is forwarded to the U.S. authorities (in particular the Department of Homeland Security) authorized to issue residence permits for the United States.
